How to apply

- Send us a request using the form on this page. Please do not send documents yet.
- One of our board members will personally reach out to you soon to talk through how we can best support you, and to explain how to send your completed application and documents.
Due to the newness of our organization at this time, we are only able to assist those residing in Sangamon County.
Whether you need help with resources, daily living support, or other forms of assistance, we are here for you.
